Not every rotator cuff tear is the same—and not ev Not every rotator cuff tear is the same—and not every tear is a good candidate for PRP.A partial-thickness supraspinatus tear may still have substantial intact tendon, making the discussion around conservative treatment, progressive loading, and potentially PRP very different from a large or retracted full-thickness tear. That’s why I want to see the tendon before recommending an injection.Diagnostic ultrasound allows me to evaluate the supraspinatus in real time: tendon thickness and architecture, the location and depth of a tear, whether fibers remain intact, and how the tendon behaves dynamically. And that information can directly change the treatment conversation.PRP may be considered in selected patients with tendinopathy or certain partial-thickness rotator cuff tears, particularly when symptoms persist despite appropriate rehabilitation. A full-thickness tear—especially a larger, retracted tear with significant weakness—requires a different conversation and may warrant MRI and/or surgical consultation rather than simply proceeding with PRP.The goal isn’t to inject every painful shoulder. It’s to understand what is actually injured, determine whether PRP makes sense for that specific pathology, and then choose the treatment most likely to get that patient back to what they want to do.Image first. Knee arthritis? Your treatment options go beyond c Knee arthritis? Your treatment options go beyond cortisone shots, surgery, or giving up your sport.I’ve been getting a lot of questions recently about PRP for knee osteoarthritis—and for good reason. In my sports medicine practice, PRP has been one of the treatments I’ve found most helpful for active patients with knee arthritis who are trying to decrease pain and keep doing the things they love.I’ve seen patients experience meaningful pain improvement lasting 6 months or longer after treatment. Results vary, but research also suggests PRP can improve pain and function for some patients with knee osteoarthritis. So what are we actually doing?PRP uses a concentrated preparation of platelets from your own blood. It is then injected into the knee under ultrasound guidance, allowing me to visualize the joint and precisely place the treatment where intended. But at Physician Coach, the injection is rarely the entire plan.We look at the knee, your strength and movement, what you’ve already tried, and—most importantly—what you’re trying to get back to.Running. Hiking. Lifting. Pickleball. Skiing. CrossFit. Or simply moving without constantly thinking about your knee. The goal isn’t to make an arthritic knee look “normal” on an X-ray.The goal is to help you hurt less, move better, and stay active longer. If knee arthritis is starting to change what you can do, PRP may be one option worth discussing.📍 Physician Coach | Tacoma, WA💉 Ultrasound-Guided PRP🔗 Book a Sports Medicine Evaluation through the link in our bio#KneeArthritis #PRPInjection #KneePain #SportsMedicine #TacomaSportsMedicine
YOUR VO₂ MAX ISN’T THE WHOLE STORY. ⚡ Especially YOUR VO₂ MAX ISN’T THE WHOLE STORY. ⚡Especially if you’re a hybrid athlete.VO₂ max tells us your aerobic ceiling. But having a high ceiling doesn’t necessarily mean you can sustain a high percentage of it during competition. For hybrid athletes, performance is heavily influenced by your ability to maintain a strong running pace while repeatedly transitioning between demanding functional movements. That’s where LT2 becomes incredibly important.Your LT2 represents the upper boundary of your sustainable aerobic intensity. Improving your pace or power at LT2 can help you maintain a faster race pace, manage fatigue, and perform more efficiently throughout competition. But here’s the part that often gets overlooked:Two athletes can have the EXACT same VO₂ max and completely different training needs. One athlete may have a high aerobic ceiling but reach LT2 at a relatively low percentage of their VO₂ max. They may benefit from developing their aerobic foundation and threshold.Another athlete may already be able to sustain a high percentage of their VO₂ max. Their next opportunity might be improving their aerobic ceiling, running economy, or race-specific durability. This is what makes our performance testing different. We combine VO₂ max + blood lactate testing to identify your:🔹 LT1 and LT2 thresholds🔹 Individualized heart rate, pace, and power zones🔹 LT1 and LT2 as a percentage of your VO₂ max🔹 Lactate response as intensity increases🔹 Biggest opportunities for physiological improvementBut we don’t stop at giving you your training zones. We help you identify WHICH zone needs your attention based on your physiology and your performance goals.Because the goal isn’t simply to have a higher VO₂ max. It’s to improve how much of your aerobic capacity you can use when it matters most.KNOW YOUR CEILING. KNOW WHAT TO TRAIN.📍 Physician Coach | Tacoma, WA🏃 VO₂ Max + Blood Lactate TestingBook your performance test through the link in our bio.#HybridAthlete #VO2Max #LactateTesting #PerformanceTesting #TacomaWA
